Statement of lived experience – access request

This template helps you put together a statement of your lived experience so the NDIA can better understand how your disability impacts you and why you need access to the NDIS. The more detail and examples you give, the better.

This template helps the people who support you explain the full impact the lack of access to the NDIS has on both you and the people who support you.

Forms

These forms are from the National Disability Insurance Agency (NDIA) and cover a range of processes.

Access request

This is the form to use if you want to request access to the NDIS either for the first time, or to make a fresh application.

Internal review

This is the form to use if you are not happy with a decision the NDIA has made and you want them to review their decision. You have 3 months to submit this form from the date you were notified of the decision.

This is the form to use if your details or situation change.
