These are useful templates our NDIS team have created for people dealing with requests and appeals for supports.
Supports in dispute
This template helps you to list the supports you need and the evidence you have submitted to back up your request so it is clearly laid out for the NDIA or AAT.

Statement of lived experience
This template helps you put together a statement of your lived experience so the NDIA can better understand how your disability impacts you and why you need the supports you are asking for. The more detail and examples you give, the better.

Carer statement
This template helps the people who support you explain the full impact the lack of proper supports has on both you and the people who support you.

Letter to expert
This template helps you request a professsional report from an expert to add to your supporting evidence. It also explains to the expert what they need to include in their report and why.

Table of evidence
This template will help you organise your evidence and check that the documents you have fully address the relevant criteria and rules. It can also be useful to give this table to the NDIA as a record of the evidence you submit.

These forms are from the National Disability Insurance Agency (NDIA) and cover a range of processes.

Access request
This is the form to use if you want to request access to the NDIS either for the first time, or to make a fresh application.

Internal review
This is the form to use if you are not happy with a decision the NDIA has made and you want them to review their decision. You have 3 months to submit this form from the date you were notified of the decision.

Change of circumstances
This is the form to use if your details or situation change.

Appeal to the AAT
This is the form to use if you are not happy with the outcome from an internal review and you want to appeal to the Administrative Appeals Tribunal (AAT).
Participant information access request
This is the form to use if you want the NDIA to share information with you about your application, plan, review, or other decisions or documents.
